What You’ll Do

As a Registered Nurse in Home Health, you will:

Deliver skilled nursing care to patients in their homes
Complete comprehensive patient assessments and develop individualized plans of care
Serve as the primary nurse for your caseload—assessing, planning, coordinating, implementing, and evaluating care
Perform initial evaluations, routine visits, and re-evaluations based on patient needs
Monitor and document patient progress, changes in condition, and outcomes
Communicate significant findings and care updates with physicians, clinical leadership, and the interdisciplinary team
Revise care plans as patient needs evolve
Ensure compliance with HIPAA and all regulatory standards

What We’re Looking For

Required:

Current, unrestricted RN license in the state
At least 1 year of clinical nursing experience
Ability and willingness to travel within the Bloomington territory (approximately 50%)
Valid driver’s license, reliable vehicle, and current auto insurance

Preferred:

Associate’s or Bachelor’s Degree in Nursing
Previous Home Health or Hospice experience
Experience with HomeCare HomeBase

Perform routine charting duties as required & in accordance with established charting & documentation policies & procedures. Prepare & administer medications as ordered by the physician. Develop & maintain a good working rapport with inter-departmental personnel, as well as other departments within the facility to ensure that nursing services and activities can be adequately maintained to meet the needs of the residents. Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift. Receive/give the nursing report upon reporting in and ending shift duty hours. Make rounds with physicians as necessary.
