What You’ll Do
As a PDN LPN, you will:

  • Provide skilled, one-on-one nursing care in the patient’s home
  • Care for pediatric and young adult clients (infant to mid-20s)
  • Manage complex medical needs, including:
    • Tracheostomy care
    • Ventilator support
    • G-tubes / feeding tubes
    • Medication administration
  • Monitor patient status and respond confidently to changes in condition
  • Partner with families, offering education and compassionate support
  • Collaborate with our clinical team to follow individualized care plans

This is focused, meaningful nursing — no rushing between rooms, no overwhelming ratios. Just you and your patient.

The Co-worker Health Nurse is a critical job that works closely with all co-workers from day one. Plans, organizes, directs and performs all activities of the co-worker health services in concert with federal and state requirements and regulatory agencies. Processes new co-workers for a health assessment at hire, which includes pre-employment drug testing, health medical history, lab work, chest x-ray if indicated and immunization status.
