Registered Nurse / RN - Discharge Plan of Care

Job Summary and Qualifications
The Patient Experience & Discharge Plan of Care Nurses responsible for ensuring that patients clearly understand their plan of care throughout their hospital stay and during the discharge process. This role bridges communication among physicians, nursing staff, patients, and families to provide one consistent, accurate message regarding treatment plans and post discharge‑ needs. The nurse supports patient experience initiatives, reduces communication gaps, and ensures safe, informed discharge practices.
Plan of Care Responsibilities:
- Collaborate daily with physicians to review, clarify, and confirm patient plans of care.
- Communicate the plan of care to patients and families in clear, understandable language.
- Reinforce communication to bedside nursing staff to ensure alignment and consistency.
- Participate in interdisciplinary rounds and provide updates on patient understanding.
- Assess patient comprehension and address questions or concerns using teach‑back methods.
- Serve as a liaison to bridge communication gaps between physicians, nursing, and patients.
- Support patient experience initiatives and help improve HCAHPS communication metrics.
Discharge Responsibilities:
- Review discharge instructions for accuracy and completeness.
- Verbally explain discharge instructions to patients and families, ensuring understanding.
- Clarify physician discharge orders with providers and nursing when needed.
- Confirm that follow‑up appointments, prescriptions, and post‑acute needs are communicated.
- Partner with Case Management and Social Work for complex or high‑risk discharges.
- Ensure documentation of all patient education and discharge interactions.
- Support initiatives aimed at reducing avoidable readmissions through strong patient education.
What qualifications you will need:
- (RN) Registered Nurse; Current TX license
- Associate Degree; BSN preferred
- Minimum 2 years of acute care nursing experience
- Experience in patient education, case management, or patient experience roles
- Advanced Cardiac Life Spt must be obtained within 30 days of employment start date
- Basic Cardiac Life Support must be obtained within 60 days of employment start date
