CentraState Medical Center has an employment opportunity available for an Advanced Practice Nurse to support the Care Coordination department. This position facilitates the discharge process to ensure a smooth transition of care.

 

Four 10-hour day shifts (7a-5p). The 1 day off is either a Tuesday, Wednesday or a Thursday. No weekends.

Responsibilities

  • Handles all responsibilities of a case manager.
  • To facilitate the discharge process, places rehab orders as needed for the transition of care as required by most post-acute facilities.
  • Facilitates discharge orders including Covid testing orders by communicating with attending physician and writing the actual discharge order.
  • Facilitates the writing of DME (Durable Medical Equipment) orders when needed for patients.
  • Furnish patients with appropriate prescription to obtain needed equipment.
  • Facilitates the writing of level of care orders. When an account does not meet the level of care that the patient was admitted for and with collaboration with the attending physician enter the level of care order in the medical record.
  • With collaboration of the attending physician, writes antibiotic and/or oxygen orders needed for post discharge.
  • Performs peer to peer appeals on post acute denials.
  • Acts as back up for the Physician Advisor handling peer to peer appeals and written appeals when necessary.
  • Performs secondary clinical assessment and documentation to support inpatient admissions as well as to support subacute, long term care(trach weaning necessity/settings) post discharge.
  • Assists denials staff with open communication and feedback to medical staff in regards to denials due to level of care and insufficient documentation.
  • Supports care coordinators to provide real time feedback to providers for improved documentation that will defend the length of stay.
  • Works collaboratively with care coordination manager in process improvement in regards to length of stay, denials and readmissions.
  • Supports care coordination department with difficult discharges and/or refusal to leave facility to a more appropriate level of care.

Qualifications

  • Bachelor's degree in Nursing required.
  • Current NJ APN license required.
  • Previous healthcare experience required.
  • Previous experience in an inpatient hospital setting preferred.

  • Experience with EPIC preferred.

  • Strong professional, organizational, and interpersonal skills to effectively relate with all members of the healthcare team.

Compensation Range: $117,000 - $198,900 annually 

 

The compensation above reflects the established range from CentraState Healthcare System (CSHS) for this position at the time the job was posted. CSHS considers many factors to determine compensation, including education, experience, skills, licenses, certification, and training.  As such, team member compensation may fall outside this range. 

 

Additionally, the compensation range reflects base salary and does not include extra shift rates or incentives tied to quality, productivity, etc., as applicable.

 

The benefits outlined also reflect CSHS’ policy at the time of posting.  Benefits as are made available to other similarly situated team members of CSHS, although participation is at all times in accordance with and subject to the eligibility and other provisions of such plans and programs.
