Job Role Summary

The RN-Specialist Palliative Care serves as clinical consultant, educator, practitioner, and care coordinator with duties to include, but not be limited to: triage, scheduling, medicine reconciliation and procurement, serving as inter- and intra- healthcare liaison to facilitate quality patient care and achieve optimal care outcomes. This position provides care/interventions to identified clients and families which may include but are not limited to: advanced care interventions/procedures, in-depth patient and family education, patient and family counseling, in-depth patient assessment, and documents in the medical record through providing care in an outpatient setting with interventions consistent with scope of licensure and professional credentialing.

Essential Functions and Responsibilities

•    Functions as a clinical practice expert providing focused professional nursing care for identified specialty in the outpatient setting
•    Participates in own professional development by maintaining clinical education requirements
•    Establishes and facilitates patient care goals with patient, family, and health care team to achieve optimal outcomes, by providing education, coordinating care, and monitoring progress
•    Collaborates with identified team to achieve organizational objectives/goals for clinical program and individual practice within area of expertise
•    Provides direct care/interventions to identified clients and families.
•    Adapts care and interventions to meet patient care needs in relation to age, size, language, and culture 
•    Self-directive, efficient and comprehensive in completion of workload
•    Utilizes established protocols to manage phone, triage of patients to identify level of care needed, triage priority, and implementation of an appropriate plan of action
•    Demonstrates effective verbal and non-verbal communication skills in working with staff, patients/families, providers, other departments and external contacts
 

Job Requirements

•    BSN from a professional school of Nursing; MSN preferred
•    Current Indiana license as a Registered Nurse
•    Minimum of five years of clinical nursing experience, 3+ of those years in area of expertise, and demonstrated expertise via progressive clinical experience

•    Demonstrates advanced nursing care/clinical expertise in nursing techniques/methods/theory, competence and accountability in relation to area of expertise
•    Can integrate evidence-based practice into own practice and any policy, procedure, standards and/or protocol review or development
•    Excellent communication skills required
•    Organize, manage and evaluate workload of self, prioritize duties and patient needs
•    Initiate, teach, evaluate planning and the provision of total patient care and effectively respond to stress

Job Summary Serves as a nurse in the Radiation Therapy Department. Will act as a resource person to both in and out patients providing quality nursing care and involvement in all phases of treatment, convalescence, rehabilitation, and follow-up of patient.
