Job Summary

We are seeking a PRN/Per Diem Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our dedicated team of healthcare professionals. As a PRN/Per Diem Nurse, you will provide high-quality patient care, work collaboratively with our multidisciplinary team, and contribute to the development and implementation of individualized patient care plans.

Responsibilities

Provide direct patient care, including assessment, planning, implementation, and evaluation of patient care plans
Collaborate with the interdisciplinary team to develop and implement individualized patient care plans
Monitor and document patient progress, report changes in condition, and communicate with physicians and other healthcare professionals
Participate in quality improvement initiatives and patient satisfaction surveys
Maintain accurate and timely records of patient care and services

Requirements

Current RN or LPN licensure in the state of Ohio
Minimum of 0 years of experience in a post-acute care setting (rehabilitation and skilled nursing care)
Must be willing to work at least one shift per month and one holiday per year if available.
