The Avenue at Medina is a Post-Hospital Rehabilitation and Skilled Nursing Care Facility that offers a distinctive environment not commonly found in skilled nursing facilities. Our facility provides some of the area’s highest level of rehabilitation, nursing and medical support to individuals needing post-hospital care and services. We are looking for a committed individual with a reputation for being cheerful and friendly as they administer care to our residents. The primary purpose of the charge nurse position is to provide direct nursing care to residents and to supervise the day-to-day nursing activities performed by nursing assistants. Such supervision must be in accordance with current federal, state, and local standards, guidelines and regulations. Responsibilities Monitor patient’s condition and assess their needs to provide the best possible care and advice Observe and interpret patient’s symptoms and communicate them to physicians Perform routine procedures (blood pressure measurements, administering injections etc.) Perform skilled charting via the point click care charting system Adjust and administer patient’s medication and provide treatments according to physician’s orders Maintain excellent hygiene and safety (decontaminating equipment, sanitizing surfaces, preparing beds etc.) Provide instant medical care in emergencies Supervise nursing assistants Foster a supportive and compassionate environment to care for residents and their families Expand knowledge and capabilities by attending monthly in-services If you are an individual who strives to provide excellent quality of care and customer service, we would love to have you join our team!

Responsibilities include: Assisting the Activities director in implementing programs of individual and group activities based on resident's needs and preferences. Assist and implement facility and community events. Accept assigned duties in a cooperative manner. Implement evening and weekend activities as assigned. Documentation of participation through Point Click Care. Qualifications and Skills required: Knowledge of Microsoft office including Excel, Word, and Outlook. Positive and outgoing personality. Previous experience in a long term care environment preferred but will consider those with other customer service experience. Flexibility in scheduling and working every other weekend is required.
