Licensed Practical Nurse (LPN) Responsibilities but not limited to:

Supports and promotes the nursing department functions on the unit by following established policies and procedures
Is knowledgeable and responds to nursing and building emergencies according to established policy and procedure.
Assigns duties and responsibilities to unit staff members in a manner that facilities optimum delivery of care to residents
Conducts regular rounds on the unit and notifies appropriate personnel of findings
Supervise and evaluate all direct resident care
Obtain report from nurse being relieved and record sufficient information to implement appropriate follow-up action as necessary
Identify safety hazards and initiate corrective action immediately
Identify resident problems and emergency situations and initiate emergency care and life-saving measures in the absence of a physician
Participate in facility education and in-service programs
Provide supervision to Nursing Assistant providing direct resident care. Provide resident care as necessary
Perform comprehensive assessment of residents as assigned
Promptly report changes in resident conditions to RN Supervisor, Physician, Director of Nursing and responsible party. Take follow-up action as necessary

Qualifications:

Active LPN license in the state of Pennsylvania
Ability to multi-task
Strong patient assessment skills
Ability to communicate effectively
Demonstrate clinical competency

All candidates must pass pre-employment screening and PA Criminal Background in accordance with the PA Older Adult Protection Act.

JOB SUMMARY Provides all necessary services needed to assist hospice patients to live in the Marquette Hospice House, becoming part of the Hospice team to carry out the patient’s plan of care. JOB DUTIES Provides direct care to guests (hospice patients) who live in the home. Assist in the facilitation of smooth patient transition, greets guests and their families courteously. Prioritizes patient care. Provides all identified needs of our guests under the direct supervision of the Hospice RN who operates under physician orders. Administers medication to guests as regulated by the nurse practice acts and in accordance with guests’ plan of care. Manages medication box and medication audits. Maintains the organized flow of nursing information and documentation. Answers all phone calls courteously and relays messages appropriately. Provides assistance with all ADL’s. Supports the hospice philosophy. Provides family education while working with the interdisciplinary team. Provides emotional support for our guests and their visitors. Responds to guest and visitor requests and complaints with immediate notification to direct supervisor or designee. Completes daily household chores to ensure home is clean i.e. laundry, dusting, sweeping, dishes, etc. Works cooperatively with the volunteers that will be helping with household chores. Maintains a safe environment for our guests and their visitors. Demonstrates knowledge of emergency procedures. Plans and prepares meals, snacks, and supplements as needed. Maintains close communication with RN supervisor regarding guest status. Notifies RN of any changes in guests’ condition in a timely manner. Performs other duties as assigned.

LICENSED PRACTICAL NURSE- Oncology - Full Time- Dayshift

Nurse would be responsible for rooming patients, obtaining vitals signs and reviewing medications. Coordination of care with testing and outside referrals Phone triage and prior auth experience desired. Expectation would be complete chart prep prior to office visit. Would serve as a conduit in patient care between physician and patient. Compassion and empathy required in this patient population. SUMMARY Provides nursing services to the patients and families, assists physician with patient care in conformance with the Licensed Practical Nurse Act. To perform all clinical functions in a physician’s office. Assisting the physician with exams, obtain specimens from patients and process properly, tracking sequential lab draws, maintaining medical information and filing/scanning, patient education, obtaining histories and vitals from patients, triage patient calls, prescription refills, call backs for normal or abnormal results as per physician direction, prior authorizations as needed, telephone answering and appointment scheduling as needed. JOB DUTIES Prepares patients for provider visits according to approved policies and procedures i.e. reason for visit, history (past medical, family and social), symptoms, problems, weight, height, vital signs, medications, allergies. Reviews and updates medication lists identifying any possible ineffective drug therapies, adverse reactions, side effects, drug allergies and contraindicated medications. Notifies patients of lab and test results. Responsible for patient safety. Communicates and interacts effectively with patients and families, physicians, peers and other health team members to maximize successful patient outcomes. Patient education. Assists in the facilitation of smooth patient flow. Greets patients and families courteously. Maintains good communication with providers and coworkers. Receives prescription refills calls and are appropriately sent to the pharmacy under a provider’s direct supervision and are thoroughly documented in the patient record by the end of the business day the prescriptions were taken care of. Assures nursing documentation in the medical records is complete . Performs administrative duties as requested. To include but not limited to: Appointment scheduling and follow up appointment scheduling as needed, telephone etiquette including message taking and recalling patients, obtain authorizations as needed, Maintains exam rooms and workstation and ensures cleanliness. Maintains the organized flow of nursing information and documentation. Documents are organized and available for use when needed. Triage patient clinical calls. Use of Meditech, ARIA and external EMR applications Demonstrates knowledge of emergency procedures. Other duties as assigned. Must have the ability to apply appropriate rules of grammar, spelling and punctuation. Must possess the ability to prioritize activities, organize work and problem solve. Must be able to deal with anxious and angry people. Must be able to communication with Administrative Staff, Managers, Attorneys, Physicians and all hospital staff with due respect and tact. Current BLS Healthcare Provider card. WORKING CONDITIONS Work setting will be in an office involving the ability to bend, stoop, lift, twist and maneuver as necessary to conduct patient evaluations. Possible exposure to various chemicals relating to job performance will be in use of equipment and in all clinical/medical procedures which may be performed. Use of computers, printers, fax machines, copiers, telephone and paper shredders. Must have ability to exhibit independent judgment skills and show initiative. Ability to use tact and poise in representing MMC. Work with minimal direction and assistance and frequent interruptions. Must be able to multitask. Must be able to work first shift but there maybe times that there will be additional hours that will be needed on short notice. Ability to manage multiple priorities under stress. Flexibility to adapt to changing situations in a positive, professional manner.
