Job Summary:

Emerald Nursing and Rehab Columbus is seeking a compassionate and dedicated Full-Time Licensed Practical Nurse (LPN) to join our team of healthcare professionals. As a FT LPN, you will have the opportunity to work with our patients in a post-acute setting, providing high-quality care and support to help them achieve their rehabilitation goals.

Responsibilities:

Provide direct patient care, including administering medications, wound care, and treatments
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Monitor patient progress and report any changes or concerns to the healthcare team
Collaborate with physical, occupational, and speech therapists to develop and implement treatment plans
Maintain accurate and detailed records of patient care and treatment
Participate in quality improvement initiatives to improve patient outcomes and satisfaction
Provide emotional support and education to patients and their families

Requirements:

Currently licensed as an LPN in the state of Nebraska
Associate's degree in Nursing or related field
1 year of experience as an LPN
Strong communication and interpersonal skills
Ability to work effectively in a fast-paced environment
Compassionate and empathetic approach to patient care
Familiarity with electronic medical records and clinical software
