Job Summary: 

As a Licensed Practical Nurse (LPN) at Emerald Healthcare, you will play a crucial role in delivering high-quality, compassionate care to our residents. Work Environment: 

Work is performed in a skilled nursing facility with potential exposure to infectious diseases and hazardous materials. Appropriate personal protective equipment (PPE) is provided. 

Responsibilities: 

  • Patient Care: Provide direct nursing care to residents, including administering medications, wound care, and treatments as prescribed by the physician. 

  • Monitoring and Assessment: Monitor residents’ vital signs, document their progress, and report any significant changes to the Charge Nurse or Director of Nursing. 

  • Care Planning: Assist in the development and implementation of individualized care plans, ensuring they are tailored to meet each resident’s needs.

  • Communication: Maintain clear and accurate documentation of patient care, communicate effectively with residents, families, and the healthcare team. 

  • Compliance: Adhere to all facility policies, procedures, and regulatory requirements, including infection control and safety protocols. 

  • Support: Provide emotional support and education to residents and their families, helping them understand care plans and health conditions. 

  • Team Collaboration: Work collaboratively with other healthcare professionals, including RNs, CNAs, physicians, and therapists, to deliver coordinated and comprehensive care. 

Qualifications: 

  • Licensure: Valid LPN/LVN license in Nebraska

  • Education: Graduation from an accredited LPN/LVN program. 

  • Experience: Previous experience in a skilled nursing facility or similar healthcare setting preferred.

  • Certifications: Current CPR/BLS certification required. Additional certifications may be required based on state regulations. 

  • Communications: Excellent verbal and written communication skills. Ability to work collaboratively with a diverse team of healthcare professionals and build positive relationships with residents and their families. 

  • Physical Requirements: Ability to lift and move patients, perform physical assessments, and stand for extended periods.
