While an interdisciplinary care team to provide medical care to patients in long term care facilities, you will have the opportunity to impact in a positive way the life and wellbeing of the elderly population.

We are currently seeking Nurse Practitioners provide medical services to patients living in long term care facilities.

NP ROLE AND RESPONSIBILITIES:

  • Provide primary and preventative care for long term care patients residing in skilled nursing facilities.

  • Manage Residents with acute change in condition.

  • Advanced care planning and chronic care management.

  • Prescribe appropriate medications, including controlled substances in accordance with NH Law.

  • Order, review and/or follow-up with treatments and diagnostic testing.

  • Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.

  • and/or the care plan.

  • Collaborate with primary care physician.

NP QUALIFICATIONS AND EDUCATION REQUIREMENTS

  • Current, unrestricted NP license in the state of New Hampshire.

  • Board Certification.

  • DEA licensure.

  • Clinical background in adult, family or geriatric specialties.

  • Experience working in a long-term care setting.

  • Ability to work with an EMR.

  • Ability to manage patients with advanced illness and multiple chronic conditions.

  • Ability to work within an interdisciplinary care team to coordinate patient care.

Description Per Diem - MaineHealth Behavioral Health – Sanford Inpatient, is seeking an experienced Psychiatric Mental Health Nurse Practitioner (PMHNP) or Psychiatric Physician Associate/Assistant (PPA) to care for adults facing acute psychiatric crises on our 40-bed inpatient unit. This is a dynamic clinical role that includes performing psychiatric evaluations, medication management, and acute care in the setting of a 40 bed adultacute inpatient psychiatric facility. You’ll be supported by 24-hour physician coverage and a culture that prioritizes both patient care and provider well-being. Previous experience required. Hours: 8:00am - 4:30pm, weekends and weekdays.
