Background Check: Must be able to pass a criminal background check (national and state-required). Communication: Excellent communication skills for interacting with clients, family members, and team members. Essential Duties and Responsibilities: Care Plan Implementation: Follow personalized care plans to ensure clients receive the appropriate support and assistance. Client Support: Provide companionship, meal preparation, housekeeping, transportation, and personal care (bathing, dressing, toileting, feeding). Communication and Advocacy: Report any changes in client condition to the Care Manager and advocate for client needs. Safety and Compliance: Adhere to safety protocols and infection control practices; notify the office if additional supplies are needed. Timekeeping and Documentation: Accurately log hours, tasks, and client status using ClearCare Telephony system. Professionalism: Maintain a professional appearance and conduct, adhering to company policies and dress code.
