Position Summary:

The Registered Nurse functions as a primary nurse and has accountability for providing quality nursing care for patients in their place of residence, as prescribed by the physician and in compliance with the standards of the Homecare Agency Infection Control Policies and HIPAA Policies. Performs and completes start-of-care, recertification’s, resumption-of-care, and discharge Comprehensive OASIS assessment. Performs all duties in a manner consistent with St. Luke’s mission and values.

Education, Experience, & Licensing Requirements:

Education: Nursing Degree

Experience: 2 years of Critical Care Experience (ICU, Emergency Services, Urgent Care Clinics)

Licensure: Current licensure as a Registered Nurse in the state of Missouri

This role delivers high-quality professional nursing care in a clinical setting, adhering to established medical standards and practices to ensure optimal patient outcomes. Shift: Full time days - 40 hours/week Key Responsibilities: Clinical Coordination: Manage patient flow, perform telephone triage, and coordinate care for emergent patient needs and appointment scheduling. Patient Advocacy & Education: Act as a subject matter expert for patient assessment and testing interpretation; identify and address learning needs for patients and their families. Clinical Oversight: Delegate tasks to clinical staff based on competency and Scope of Practice; oversee the management of expired clinical samples. Interdisciplinary Support: Maintain a professional demeanor while working both autonomously and collaboratively.
