About the Role
The Wound Nurse plays a critical role in providing specialized wound care and promoting optimal healing outcomes for residents. As a clinical expert and member of the interdisciplinary team, the Wound Nurse is responsible for assessing, treating, and monitoring wounds while ensuring compliance with clinical standards and regulatory requirements. This position requires strong assessment skills, attention to detail, and a commitment to high-quality, resident-centered care.
Key Responsibilities
Conducts comprehensive wound assessments and develops individualized wound care treatment plans.
Provides direct wound care treatments in accordance with physician orders and evidence-based best practices.
Monitors wound healing progress and promptly reports changes in condition to the interdisciplinary team and physicians.
Ensures accurate and timely documentation of wound assessments, treatments, and outcomes.
Collaborates with nursing staff, physicians, therapists, and dietitians to support holistic wound management.
Educates nursing staff, residents, and families on wound prevention, treatment protocols, and skin integrity best practices.
Participates in skin rounds, quality assurance initiatives, and infection prevention efforts.
Assists with MDS documentation related to skin integrity and wound care as applicable.
Ensures compliance with federal, state, and local regulations, as well as facility policies and procedures.
Supports pressure injury prevention programs, including risk assessments and staff education.
Performs additional duties as required to support resident care and facility goals.
Qualifications
Current RN license in good standing in the state of Pennsylvania.
Minimum of 1–2 years of nursing experience in a long-term care or skilled nursing setting preferred.
Wound care certification (WCC, WOCN, or equivalent) preferred but not required.
Strong knowledge of wound care products, treatments, and best practices.
Excellent assessment, documentation, and communication skills.
