Overview

Plans and manages patient care according to each patient's needs. Interviews patients and records their medical history and physical condition. Obtains patient vital signs, including pulse, blood pressure, temperature, and respiration. Provides routine care for patients.

Responsibilities

  • Provide 1:1 care for client always following client’s plan of care while maintaining client's dignity and independence
  • Monitoring clients’ vital statistics and reporting abnormal findings to the case manager and physician assessments
  • Performs medication administration, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
  • Complete routines as established by other care professionals such as Physical Therapist, Occupational Therapist, and Speech Therapist
  • Ensures infection control policies are maintained at all times, reports possible communicable diseases or infections, and ensures the overall implementation and practice of infection control procedures are implemented
  • Accurately documents nursing actions of all care given and communicates with family, case manager, and physicians as well as documents the progress and outcomes for established goals and informs physician, case manager, and families of changes in client's medical condition and needs
  • Orders supplies and medications, as needed, to ensure adequate amounts are on hand at all times
  • Ensures all patient needs are being met in accordance with the instructions of the physician.
  • Monitors incidents involving individuals. Ensures incident reports are completed timely, accurately, and case managers are informed; examines the individual following the incident
  • Ensures all medical equipment is maintained and reports faulty equipment to Durable Medical Equipment for repair
  • Other duties as assigned

Qualifications

  • Completed post-secondary coursework to facilitate licensure as a Licensed Practical Nurse or Licensed Vocational Nurse
  • Knowledge of individual service plans, including understanding of client rights and health needs
  • Proof of current license as a Licensed Practical Nurse or Licensed Vocational Nurse in good standing from the state(s) of practice
  • CPR Certified
  • Excellent interpersonal skills, including customer service, patience, empathy, and compassion
  • In-depth understanding of patient care policies and regulations
  • Familiarity with symptoms and signs of improvement for several medical conditions
  • Moderate amount of travel between facilities may be required

Our Company ResCare Community Living Overview Works under the supervision of a Registered Nurse. Duties include providing routine care, general observation of client’s health status, assists the RN and physician in treatments and communicating with clients and their families/guardians. Reports changes in condition and/or additional client needs to supervisor. Responsibilities Communicates health/nursing needs and/or recommendations to RN supervisor Coordinates admissions, discharges, and follow-up as needed Assists in medical/health appointments as needed Ensures the provision of nursing/health services under the direction of the RN and as ordered by the physician, provides direct care as outlined in the Service Plan/Nursing Care Plan Takes and records vital signs, weights, wound measurements, etc. Identifies trends outside of parameters, or in need of attention and communicates to RN/physician Reports pertinent information and changes in individual health status to attending physician/health professionals and to the Team as needed in a timely manner Ensures the provision of needed adaptive equipment Medication Administration: Administers medications, verifying the correct medications are administered (per the six “rights” of medication administration), and that client’s response to medications is noted and documented along with any noted side effects Works closely with home/facility/pharmacy staff in consulting on psychotropic medications, side effects, and medication reviews Orders, reviews, monitors, and discontinues medications as ordered by physician.
