Identify eligible Medicare patients for AWV. Coordinate outreach and scheduling for AWV’s. Support providers with required documentation elements for AWV completion and compliance. Track completion rates and performance metrics. Partner with providers, leadership and staff to improve quality outcomes. Required Qualifications Education: Current LPN licensure in the State of Ohio Licensure/Certifications: BLS certification within 30 days of employment is required.
