LPNs who work at Northeast Center gain knowledge and skills that advance their careers while working in a supportive environment LPN Charge Nurse Responsibilities: Med-Cart Pass on Units Ensure all nursing personnel follow written P/P on the unit. Ensure nursing staff follow their respective job descriptions. Perform administrative duties such as completing transfers, monthlies, nurse’s notes, reports, etc. as necessary. Admit, transfer, and discharge residents as required. Complete accident/incident reports, as necessary. Participate in developing the agenda for care plan committee meetings. Ensure that rooms are ready for new admissions. Greet newly admitted residents upon admission. Escort to room, as necessary. Do nursing assessments. Make rounds with a physician. Monitors and transcribes physicians' orders, consults and appointments, as necessary. Provide direct nursing care, as necessary. Notify the neighbor’s attending physician and next of kin when there is a change in the resident’s condition. Make independent decisions concerning nursing care. Give/receive the nursing report upon reporting in and ending shift duty hours. Administer professional services such as, but not limited to catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, etc… Participate in the orientation of new neighbors/family members to the facility. Review the neighbor’s chart for specific treatments, medication orders, diets, etc., as necessary. Obtain sputum, urine and other lab tests as ordered. Take and record blood pressure, etc. as necessary. Monitor seriously ill neighbors, as necessary. Administer medications and treatments and document same as necessary. Responsible for knowledge of all safety and fire regulations. Attend Inservice programs, both in and out of facility, to increase professional competence. Responsible for knowledge of all personnel policies. Write, evaluate, and update care plans. All other duties as may be assigned. LPN Charge Nurse Requirements: Active LPN License in the state of NY Must have recent physical and MMR vaccination Experience as a charge nurse is a plus Must be willing to work med cart Compensation: Rate will be commensurate with experience. Duties & Responsibilities: As an RN Unit Manager, you will: Responsible for the completion of the MDS nursing section. May be responsible to work on unit, do medication passes. Participates in Manager on Duty, a weekend on-call program Participates in weekly plan of care meetings, promotes the interdisciplinary team approach. Keeps ADON/DON informed of neighbor status through verbal and/or written communication. Performs regular clinical rounds of neighbors and nursing staff. Monitor neighbors who have episodic illness. Assists nurses with admissions, transfers and all emergency situations including, but not limited to, technical procedures when necessary. Assists physicians as needed. Performs a total physical assessment of all critical neighbors. Makes sure that all documentation regarding the death is complete. Recognizes unsafe and unclean conditions and implements corrective action to provide neighbors and personnel with a healthy, clean, and safe environment. Ensures timely and proper handling of accident/incident reports and checks to see that documentation is accurate/complete and that proper follow up is performed. Oversees the management of the unit. Reviews charts for completion and documentation accuracy. Ensures correct narcotic count and documentation of controlled substances on unit. Ensures that O2 equipment, enteral feeding pumps, suction machines and other specialized equipment is functioning and that needed supplies are available. Deals effectively, professionally, and tactfully with neighbors and family complaints. Observes undesirable visitors within the facility and assists in maintaining internal and external security. Maintains staff morale and promotes their working together as a unified team. Prepares, reviews, and submits written documentation to the ADON/DON on personnel’s performance and any disciplinary action taken for conduct contrary to facility policy. Responsible for supervising, evaluating, counseling and disciplining personnel on assigned unit. Assists in orientation and on the job training of new employees and ongoing education of staff. Performs direct neighbor care and other job-related duties as required. Requirements for a RN Unit Manager: Valid RN License One year of long term care experience is required Must be in good standing with the State Registry Excellent Work Ethic Must have demonstrated the ability to make decisions with good nursing judgement, handle emergencies and maintain excellent patient assessment skills. RESPONSIBILTIES: Employee Health Services: Conduct pre-employment health screenings, including vital signs, TB testing, and immunization reviews. Administer vaccines, including flu shots and other required immunizations, under the direction of the Occupational Health RN or physician. Provide basic first aid for workplace injuries and illnesses and refer employees to appropriate medical care providers as necessary. Monitor employees returning to work after illness or injury, ensuring compliance with clearance protocols. Infection Control and Monitoring: Support infection control efforts by monitoring employee health for symptoms of communicable diseases. Conduct routine screenings for tuberculosis and other infectious conditions as required by facility policies. Educate employees on infection prevention measures, including hand hygiene and PPE usage.
