Job Summary and Responsibilities
As a Clinic Registered Nurse (RN), you will be a central figure in our patient care team, delivering exceptional, compassionate care that truly makes a difference in our patients' lives.
Every day you will collaborate on comprehensive screenings, conduct critical risk assessments, provide vital preventative health education, and expertly coordinate care for diverse patient populations, leveraging technology to streamline processes. You'll ensure patients feel informed and at ease, patiently guiding them through instructions and addressing their concerns.
To be successful in this role, you will combine robust clinical knowledge and technological proficiency with genuine compassion, strong communication skills, and an adaptable approach to learning and patient-centered care. Some duties, responsibilities & general expectations of this position are as follows:
- Demonstrates the ability to safely and competently perform tasks, skills, duties, procedures and responsibilities according to Medical Center Policy and Procedure. Including, but not limited to:
- Demonstrates the ability to work as part of the clinic's interdisciplinary team.
- Documents in accordance with SMMC policy and procedure in a timely and concise manner the following information: Assessment Intervention Patient response and outcome Unusual occurrences
- Responds to all patient phone calls: Scheduling patients with an acute illness for an acute provider visit Provides appropriate patient counseling and education. Communicates with physician any changes in patient's condition as indicated
- Encourages active participation and promotes independence of the patient in decision making related to their plan of care.
- Completes all tasks/orders as requested by providers and consistently documents outcome in a timely manner. Notifies provider if unable to contact patient to complete assigned task.
Job Requirements
- Experience & knowledge of working with various behavioral risk groups including active drug users & LGBQT community.
- HIV/AIDS related clinical experience in an outpatient clinic.
- Registered Nurse CA
- Bachelors in Nursing preferred
