About the Role
We’re looking for a dedicated Registered Nurse Assessment (RN Assessment) to support patients and families as they enter hospice care. This role is essential in ensuring a smooth and compassionate start to the hospice journey. You’ll complete initial assessments, develop individualized care plans, and provide education and support during the admission process. You’ll collaborate closely with our interdisciplinary team to make sure patients receive care that aligns with their wishes, goals, and needs.
How You’ll Make a Difference
Initial Assessments & Care Planning
Complete intake assessments for referred patients and coordinate all needed follow-up with families, physicians, insurance representatives, and hospice staff.
Assess medical, physical, emotional, and social needs and form nursing diagnoses based on your findings.
Develop individualized care plans in collaboration with the interdisciplinary team.
Oversee and case manage all bed transitions at WMC, ensuring smooth coordination and appropriate care.
Patient & Family Support
Educate patients and families on hospice services, symptom management, and care expectations.
Provide emotional support in a way that strengthens patient and family coping.
Advocate for patients to ensure their goals and preferences remain central to the care plan.
Clinical Excellence
Implement nursing interventions focused on comfort, pain management, and symptom relief.
Provide hands-on teaching to families, caregivers, and home health aides.
Follow universal precautions and isolation techniques as required.
Maintain accurate, timely clinical documentation for all assessments, referrals, and interventions.
Team Collaboration & Community Engagement
Serve as a liaison nurse with WMC for patients in transition.
Respond to questions from individuals contacting hospice and serve as a referral resource.
Participate in training and orientation of new team members.
Educate staff in other care settings on hospice philosophy and pain/symptom management.
Work collaboratively within the interdisciplinary team and identify when other services (social work, chaplaincy, etc.) are needed.
Professional Standards & Responsibilities
Maintain patient confidentiality and autonomy at all times.
Seek ongoing opportunities for professional growth and accept constructive feedback.
Recognize when family systems or circumstances are not amenable to change and adjust care plans accordingly.
Act independently, responsibly, and in alignment with the mission and values of Blue Ridge Care.
Share weekend and holiday rotation with the Intake Team.
Maintain an active driver’s license and reliable, insured transportation to travel across the service area.
Other duties as assigned.
What You Bring
Graduate of an NLN-accredited nursing program; BSN preferred.
Active RN license in the state of Virginia.
At least one year of medical-surgical or oncology experience required.
Minimum of six months hospice home care experience preferred.
Proficiency with EMR systems, including Epic.
Strong communication and assessment skills, with a compassionate and patient-centered approach.
