A Place at Home Towson is looking to hire an awesome per diem Registered Nurse (RN) to be responsible for the case management of Clients and ensuring the highest level of quality care in the Baltimore area and that wants to be a part of a GREAT TEAM!

Job Duties:

Performing comprehensive assessments of client status, including physical, psychosocial, and environmental parameters.
Ensures that all safety and infection control practices are followed
Responds to emergency situations
Communicate with other members of the home health team
Ensures that required documentation is complete and in compliance with state and accrediting standard
Communicate with patients and families regarding treatment and care
Developing an individualized plan of care for each client that incorporates analysis of data and current scientific findings.
Open new cases by assessing client needs and, writing Care Plans
Case monitoring, conduct 60-day client visits and 30-day client check in phone calls
Providing interventions with a focus on achieving realistic client outcomes.
Reporting changes to update the plan of care to reflect progress towards goals and outcomes.
Administering medications as needed.
Maintaining client records showing systematic assessments, planning interventions, and evaluations.
Initiating referrals as needed to other health team members.

QUALIFICATIONS:

Must be a Registered Nurse in the State of Maryland for at least two years and hold an unencumbered license

Two years of progressive community health, public health or home care experience, two to four years preferred

One year experience supervising homemaker-home health aides, preferred

One-year experience in geriatric care, preferred

Ability to supervise, guide and develop skills and performance of service personnel

Ability to exercise independent judgment

Education/Skills Requirements:

- Registered Nurse degree from an accredited institution.

- Willing to undergo a background check
