Responsibilities

Responsible for assisting in the delivery of resident care through assessment, planning, implementation, and evaluation
Review resident medical records to ensure nursing practice provided is within State and Federal guidelines
Review all referrals for consultation, treatment or resolution of problems are valid and applicable to the documented nurse plans
Evaluate and monitor resident responses to daily implementation of nursing care to ensure the provision of resident care plans are met
Implement physician’s orders, nursing interventions and procedures as needed for the care of the resident
Seeks opportunities for professional growth and expansion of responsibilities to maintain competency levels
Set and implement goals and policies consistent with the philosophy and purpose of FutureCare’s nursing facility

Qualifications

Must hold an active Registered Nurse license issued in the state of Maryland
1 year of nursing experience preferred
Previous experience caring for residents in an acute or long-term health care facility a plus
Must possess a current CPR license

GENERAL RESPONSIBILITIES Participates in prospective residents' admission process in terms of their nursing needs and appropriate placement. Determines potential Resource Utilization Groups (RUGS) and expense associated with a potential admission. Reviews all applications for admission, followed by on-site or phone assessment and communicate results of assessment to the Admissions Coordinator. Complete and assure the accuracy of the MDS process for all residents. Maintain current working knowledge of Medicare criteria, serving as a resource for nursing staff and communicate changes in regulations. Monitor Case Mix Index (CMI) scores, looking for potential risks and/or changes that may affect Medicaid Reimbursement. Facilitate problem-solving for complicated admissions. Monitor Medicare assessment schedules and nursing documentation to ensure accuracy and timely submission. Develops care plans and assessment schedules for residents. Utilizing the RAI, MDS 3.0, QM, CBQM, and MBP manuals to inform activities and assessments. PREREQUISITES A bachelor's degree in nursing along with a valid state nursing license. Medical terminology knowledge. Exceptional mathematical skills and ability to apply them to tasks such as analysis of variance and sampling theory. Previous experience in a clinical and healthcare setting; experience as an MDS coordinator preferred. An understanding of the MDS 3.0 assessments and procedures. Knowledge of state and federal regulations. A solid understanding of data collection, reporting, and analysis. PREFERRED SKILLS Excellent leadership skills.
