NURSE PRACTITIONER (FULL-TIME, DAYS)

$100,000 - $115,000 / year

Provide Primary Care services to members of the South Central School Trust and their dependents and also provide Transitional Care visits to patients being discharged from JMH.  This position will be housed at the Immediate Care/Occupational Health Center.  

Examine and treat patients under supervision of physician within the appropriate scope of practice, including, but not limited to: performing telephone screenings of patients referred from physician offices prior to appointment being made, obtaining patient histories and performing physical examinations, ordering and/or performing diagnostic and therapeutic procedures, formulating a diagnosis, developing and implementing a treatment plan, monitoring the effectiveness of therapeutic interventions, offering counseling and education to meet patient needs, analysis of vital signs and tests as well as exam findings and making appropriate referrals. Works under the direction of the supervising physician.

Maintain documentation of all encounters with patients either by phone or face-to-face, including history and physical exams, hospital subsequent visits, plan of action, medical necessity, tests and treatments.


Code appropriately according to documentation and medical decision making of visit.


Prescribe medications with review by supervising physician in accordance with Nurse Practitioner rules and regulations in the state of Indiana.


Log and track charts for supervising physician in accordance with Nurse Practitioner rules and regulations in the state of Indiana.


Perform standard clinical procedures such as venipuncture, IV therapy, EKG, injections, removal of sutures, collection of specimens for cultures, removal of foreign bodies and taking vital signs.


Participate in hospital medical education programs as well as all other hospital education and compliance requirements.


Conduct all activities in compliance with applicable laws, regulations, standards and JMH policies and procedures including Blood and Body Substance Precautions.


Participate in organizational quality improvement activities as necessary.


Perform other duties as assigned.



EDUCATION, EXPERIENCE AND TRAINING: Current Indiana license as a registered nurse, BSN and national Nurse Practitioner Certification or MSN required. Must obtain Indiana APN-Prescriptive Authority within 90 days of employment. Current BLS certification required. Current ACLS certification preferred. CME and additional training and/or experience preferred.


The schedule will be 4 patient facing days per week and 1 administration day.

Job Description: Job Summary: The incumbent of this position is responsible for ensuring quality services are delivered to clients/patients through providing clinical oversight to clients/patients and team members in a program by planning, organizing, staffing, and directing client/patient care on a daily operational basis.
