Duties Description
The Registered Nurse 2 (Community) position will provide administrative oversight to assigned Family Care Homes. The Family Care RN 2 works within the framework of a treatment team providing nursing services to individuals with developmental disabilities, and their Caretaker/Family Care Providers. The Family Care RN 2 provides clinical oversight of the individuals health care services, conducts on site visits, develops plan of nursing service, provides nursing care, provides guidance, and makes recommendations based on the individuals medical needs for emergency or continuing care, suggests alternative treatment plans, and teaches the Family Care Provider (FCP) methods of dealing with emergencies. The Family Care RN 2 acts as a liaison between the individual, Family Care Provider, and other members of the treatment team.They also ensure that the health care needs of the individuals are provided and work with appropriate community agencies as necessary to ensure a coordinated team treatment plan. The RN 2 provides guidance and training to the Family Care Providers in dealing with emergencies. Participates in assessing the Family Care Provider’s skills and abilities in addressing the needs of the individuals. The Family Care RN 2 will be assigned to Family Care homes located in Dutchess County area and may provide temporary coverage to homes in other counties.. The incumbent will be based out of the Firemen’s Way office.

Minimum Qualifications
Applicants must possess a license and current registration as a registered professional nurse in New York State; and two years of post-licensure clinical nursing experience. One year of this experience must have been providing direct services to individuals diagnosed with mental illness, developmental disabilities, addiction, or substance use disorders in a community setting.
Substitution:
A bachelor's degree in nursing may substitute for one year of general post-licensure clinical nursing experience; and a master's degree in nursing may substitute for two years of general post-licensure clinical nursing experience.
Transfer:
Candidates must have one year of permanent competitive or 55-b/c status in the posted title and be eligible for lateral transfer; OR be eligible to transfer via Section 70.1, 70.4, or 52.6 of Civil Service Law. The transfer cannot be a second consecutive transfer resulting in an advancement of more than two salary grades.

This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.
At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status.
° Thirteen (13) days of paid vacation leave initially.
° Five (5) days paid personal leave each year.
° Thirteen (13) days of paid sick leave each year (PEF/CSEA), which may carry over from year to year.
° Up to three (3) days professional leave each year to participate in professional development events.

- Ensures compliance with the Taconic DDSOO Policy/Procedures and regulatory requirements.
- Acts as a liaison between the Individual, Family Care Provider, involved families, and other members of the treatment team.
- Communicate the individual’s health condition/care to treatment team.
- Evaluates the individual’s health care needs and treatment plan for effectiveness and make recommendations for emergency and/or continuing care.
- Communicates and coordinate care with community agencies including day programs to review treatment plan and adjusts, as necessary.
- Participates in assessing the Family Care Provider’s skills and abilities in addressing the needs of the individuals.
- Provides guidance and training to the Family Care Providers in dealing with emergencies/routine medical care.
- Provides training, education and support to the Providers and the individuals regarding health care issues and preventative health strategies.
- Participates in the assessment of the home’s physical environment and develops recommendations such as environmental modifications as appropriate to meet the individuals needs based on the environmental assessment.

- Posted Hiring Salary includes the sum of the statutory hiring rate for PS&T Grade 20 ($74,193), Increased Hiring Salary ($9,980), Geographic Pay Differential ($35,000), and Mid-Hudson adjustment ($2,000). Geographic Pay Differential and Mid-Hudson adjustment are added to the job rate ($94,121).
- If you are currently a New York State employee serving in a permanent competitive qualifying title and eligible for a transfer via Section 70.1 of the Civil Service Law, you may be appointed to this title as a permanent competitive employee.
