Unit Description: The Case Manager (CM) coordinates the care and services of selected patient populations across the continuum of illness; promotes effective utilization and monitoring of health care resources; and assumes a critical role with an interdisciplinary team to achieve optimal patient-centered, clinical, and resource outcomes. They proactively identify and intervenes to address barriers to treatment, health outcomes, and discharge planning, including biopsychosocial and other high-risk factors. They are an essential link between the patient, the healthcare team, the payer, and the community.

How you make a difference The RN Travel supports patient care delivery through assessment, planning, implementation, and evaluation under the supervision of Charge Nurses. This role involves administering medications, assisting in diagnostic tests, documenting care, and supporting surgical procedures as needed. The nurse ensures compliance with safety protocols, medication handling, and clinical documentation standards. They participate in care planning, emergency response, and interdisciplinary communication. The role requires 100% travel to various locations to fulfill staffing needs.
