RN Home Health Visit Nurse - Full Time Daylight
Location: New Stanton, PA

Schedule:

  • Monday-Friday: 8:00 AM - 4:00 PM

What You'll Do as an RN:

  • Conduct skilled nursing visits in patients’ homes (assessment, reassessment, medication administration, IV therapy, and treatments)

  • Develop and implement individualized care plans

  • Educate patients and caregivers on managing conditions and medications

  • Collaborate with a multidisciplinary team to ensure continuity of care

  • Document visits and patient progress accurately and timely


What We're Looking For:

  • Current RN license in Pennsylvania

  • At least 1 year of clinical nursing experience (home health experience preferred)

  • CPR certification

  • Valid driver’s license and reliable transportation

  • Strong communication, time management, and documentation skills

  • Passion for patient-centered care
