A skilled nursing facility located in Yonkers is seeking an experienced Per Diem Nurse Practitioner to join our team.

The Nurse Practitioner (NP) will be responsible for working in collaboration with the medical director and attending physicians in the medical management of residents admitted to the Skilled Nursing Facility/Sub-Acute Rehab unit. The Nurse Practitioner will assess, diagnose and treat the residents, according to their scope of practice and NYS Law.

The Nurse Practitioner will take/review patients medical histories, conduct physical examinations, write admission and follow up notes in the EMR, order and interpret labs and diagnostic studies, diagnose and treat medical conditions, make referrals when needed and prescribe medications including controlled medications.

Minimum Requirements

  • Maintains current nursing and advanced practice license in compliance with State law
  • Maintains current DEA licensure
  • Ability to develop and maintain collaborative relationships
  • Experience in working with older adults with complex medical conditions
  • Ability to assess subtle changes in health conditions of the older adult
  • Working knowledge of State and Federal Long Term Care regulations
  • Excellent administrative, organizational and verbal skills
  • Effective communication skills with the geriatric population
  • Ability to work independently
  • Detail oriented
  • Dependable and reliable
  • Proficiency in EMR systems (Sigma/MatrixCare a Plus)

Preferred Qualifications

  • A clinical background in adult, family or geriatric practice
  • Past experience working in a nursing home or with older adults in other settings
  • Experience in working in a post-acute setting
