Provides prescribed medical treatment and personal care services to patients in an inpatient setting by performing the following duties supporting the nursing philosophy. May include day, evening, night, weekend, holiday, and/or on-call hours. Performs other duties as assigned.

Responsibilities

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Provides quality patient care by performing treatments and procedures according to established protocols.
  • Observes patients and reports adverse reactions to medication or treatment
  • Administers specified medications as allowed by policy and procedure, licensure, and State Practice Act.
  • Responds in a safe and timely manner to patient requests and reports significant observations to the RN promptly.
  • Demonstrates knowledge of the patient's plan of care and delivers care safely and within established standards and guidelines. Participates in planning for patient's care by providing input to the RN and under the direction of the RN, assists in implementing the patient care plan.
  • Demonstrates knowledge of established hospital and nursing policies. Identifies emergency situations, initiates nursing measures as appropriate within scope of practice in emergency situations and seeks assistance and direction as necessary.
  • Instructs patients in self-care according to the teaching plan and the directions of the RN. Documents this aspect of the patient's care.
  • Participates in QA & I.

EDUCATION AND EXPERIENCE

Graduate of an accredited LPN Program OR

Wisconsin: Have completed the portion of the RN Program needed to obtain a certificate of completion in practical nursing.

Illinois: Have completed the designated curriculum of a Ladder Option Approved Designated RN Program.

Experience in clinical setting preferred.

 

CERTIFICATION AND LICENSURE

Licensed as a Licensed Practical Nurse in state of practice

Approved CPR or BLS certification required within 90 days

JOB SUMMARY Responsible to provide direct nursing care to the customer, and to supervise the day-to- day nursing activities performed by the nursing assistants. Chart nurses’ notes in an informative and descriptive manner that reflects the care provided to the customer, as well as the customer’s response to the care. Complete required assessments upon the customer’s admission, transfer, and/or discharge. Receive telephone orders from physicians and record on the Physicians’ Order Form and P.O.S. Record new/changed diet orders. Forward information to the Dietary Department. Perform routine charting duties as required and in accordance with our established Charting and Documentation Policies and Procedures. Responsible for completion of admission, discharge, and transfer processes. Prepare and administer medications and treatments if appropriate as ordered by the physician. Review medication record for completeness of information, accuracy in the transcription of the physician’s order, and adherence to stop order policies. Administer professional services such as: catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, care of the dead/dying, etc., as required. Participate in the orientation of new customers/family members to the facility. Arrange for diagnostic and therapeutic services, as ordered by the physician. Notify the customer’s attending physician and family when there is a change in the customer’s conditions. Obtain sputum, urine and other lab tests as ordered. Take and record TPR’s, blood pressures, accuchecks, etc., as necessary. Contact the customer’s physician for: Nursing assessment of change of condition. Medication orders or charges. To inform of incidents. To report abnormal results of Lab and X-ray. Anything else pertinent to the customers’ care and well-being. Ensure the nurses’ notes and monthly summaries reflect the care plan and is being followed. Attend staff meetings, etc., as directed. Perform other related duties as assigned.
