Purpose: 

We are looking for an experienced Nurse Practitioner to provide primary care to homebound and palliative care patients. You will apply medical techniques and principles under the supervision of a physician and you will care for a diverse set of patients with chronic diseases. 

What You'll Do:  

  • Provide high quality care to primary and palliative patients in and around Monmouth County, NJ
  • Assess patient's health by performing physical examinations; obtaining, updating, and studying medical histories and creating the best plan for care.
  • Effectively communicate with office staff and clinical operations across various internal platforms
  • Provide high quality, patient centered care to a panel of patients `
  • Attend quarterly medical staff meetings
  • Develop impressions, diagnosis, and treatment plans
  • Foster trust and consult with patients, families, and caregivers during visits
  • Order diagnostic testing as medically necessary
  • Prescribe medications supplies and appropriate medically necessary interventions

What You'll Need:  

  • Board Certified, with a valid NP certification in the state of New Jersey
  • Strong clinical skills with the ability to accurately and confidently treat, assess, and diagnose
  • Positive demeanor and strong people skills
  • Ability to confidently problem solve and operate independently
  • 1-2 years of primary care experience with geriatric patients (hospice experience highly preferred but not required)

Rutgers, The State University of New Jersey, is seeking a Health Care Child and Family Nurse for the Child Health Program/Child and Family Nurse Program within the School of Nursing. The Child and Family Nurse (CFN) functions to support the Department of Children and Families (DCF)/Child Protection and Permanency (CP&P) in meeting the health care, protection and well -being needs of children residing with their biologic parents. Among the key duties of this position are the following: Case conferences all Consultation Requests received with CP&P casework staff to determine appropriateness of referral to the CFN, clarify nursing needs and to inform CP&P of the CFN’s scope of involvement. Assesses child and family health care status and needs by reviewing New Jersey Spirit (NJS), New Jersey Immunization and Information System (NJIIS), Health Focus, and available medical records, and interviewing healthcare providers and biologic parents. With parental agreement completes joint home visits with caseworker. Attends pediatric subspecialty appointments as indicated. Supports the health plan developed by the primary medical home in collaboration with specialists, biologic parents, other community service providers and DCF/CP&P. Assist the division to understand strengths, barriers to care and impact of medical mental health of caregivers on their ability to support child’s medical health plan with multifaceted, interdisciplinary recommendations to address deficits.

Rutgers, The State University of New Jersey is seeking a Research Project Manager for the Rutgers Center for State Health Policy (Center/CSHP) within the Institute for Health, Health Care Policy, and Aging Research (IFH), a unit of Rutgers Health. This position requires advanced quantitative and qualitative research and project coordination skills and will support the Center's broad health services research portfolio. The selected candidate will receive strong policy and research mentorship from the Executive Director and a talented team of faculty and research staff at the Center. The Research Project Manager will be given the opportunity to participate in policy meetings with senior state officials, as appropriate, and will be introduced to a broad array of health policy stakeholders in New Jersey. The Research Project Manager will develop and implement statistical analyses and modeling of exposures, treatments, and healthcare outcomes, serving as a core member of investigative teams for several major health services research studies including the NJ Medicaid 115 Waiver Demonstration and providing statistical expertise and support to investigative teams. The Research Project Manager develops and manages project workplans for carrying out analyses of data and provides technical advice to faculty researchers. The position requires high-level skills in statistical analysis, as well as expertise in research methodology including epidemiological and health services research methodology.
