Duties Description

Do you have nursing experience working in a community setting? Manhattan Psychiatric Center is recruiting a Registered Nurse 2 Community to work with individuals with mental illness to prepare them for discharge to a skilled nursing facility. Candidates from outside or within State Service can apply.

As Registered Nurse 2 Community at Manhattan Psychiatric Center, you will be part of the Discharge Planning Team as the nurse expert and duties will include:

  • Coordinating meetings with family members to address barriers related to referrals and admissions of individuals to skilled nursing facility
  • Developing and overseeing agreement between Kingsboro Psychiatric Center and skilled nursing facility to provide convalescent care
  • Conducting site visits to skilled nursing facilities
  • Coordinating treatment and developing individualized care plans
  • Identifying other supports and personal items to assist the individual living at the skilled nursing facility

Minimum Qualifications

Candidates from outside and within State Service may be considered for hire under the Hiring for Emergency Limited Placement - Statewide (NY HELPS) program as a Registered Nurse 2 Community if they have:

  • License and current registration as a registered professional nurse in New York State, and
  • 2 years of post-licensure clinical nursing experience;1 year of this experience must have been providing direct services to individuals diagnosed with mental illness, developmental disabilities, addiction, or substance use disorders in a community setting.
  • Substitution: Bachelor’s degree in nursing can substitute for 1 year of post-licensure clinical nursing experience; and a master’s degree in nursing can substitute for 2 years of post-licensure clinical nursing experience. Neither degree substitutes for the required 1 year of specialized community nursing experience.

OR

Candidates from within State service may be considered for transfer to a competitive class position if they have one year of permanent competitive or 55-b/55-c service in a title eligible for transfer under Section 70.1 of the Civil Service Law.

Posted hiring rate includes the sum of the statutory hiring rate for PS&T Grade 20 ($74,193), an Increase Hiring Rate ($9,980), Location pay (4,000) and Geographic Pay Differential ($35,000). The posted job rate is the sum of the statutory job rate ($94,121), location pay ($4,000) Geographic Pay Differential ($35,000).

Principal Accountabilities JOB DESCRIPTION Collaborate closely with physicians and providers to support clinical decision-making and coordinate patient care. Conduct telephone and in-office triage, assessing patient symptoms, prioritizing care needs, and providing appropriate guidance. Communicate effectively with patients and families, including answering clinical messages, providing education, and relaying provider instructions. Collect, assess, and evaluate patient health data to develop and implement care plans. Ensure care is delivered in accordance with the NJ Nurse Practice Act and established protocols. Utilize EMR and other systems to document care accurately and efficiently. Participate in shared governance and contribute to process improvements focused on patient safety, quality, and efficiency. Qualifications QUALIFICATIONS Graduate of an accredited Nursing Program and valid NJ RN license required. BSN preferred. 3-4; years of experience in an outpatient or ambulatory setting; 2-3; years in acute care Strong communication, organization, and computer skills.
