Qualifications of the Nurse Practitioner:

  • Active Nurse Practitioner license in the state of Pennsylvania (required).
  • Experience working in long-term care, subacute rehab, or post-acute environments strongly preferred.
  • Strong communication and clinical decision-making skills.
  • Ability to work independently across contracted facilities.

 

Responsibilites of the Nurse Practitioner:

  • Provide clinical assessments, ongoing medical management, and patient-centered care in skilled nursing and long-term care settings.
  • Collaborate with physicians and interdisciplinary teams to support rehab goals and continuity of care.
  • Document patient encounters in accordance with regulatory and company standards.
  • Support quality initiatives and contribute to a consistent level of excellence across assigned facilities.
