JOB DESCRIPTION

Under the supervision of the Clinical Supervisor, the Registered Nurse or Licensed Practical Nurse is responsible for delivering care to clients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation.  

The RN or LPN effectively interacts and provides information on health maintenance and disease prevention, and provides case management with clients, significant others, and other health team members while maintaining standards of professional nursing.

RESPONSIBILITIES


  1. Assesses the physical, psychosocial, and environmental status of assigned clients in a systematic and continuous manner to provide required client services
  2. Consults with physicians, other staff, client/family, and other caregivers to formulate a plan of care and coordinate delivery of services to ensure high quality client care
  3. Provides nursing services in accordance with the plan of care and utilizing the nursing process
  4. Performs ongoing assessment of client’s status and response to services to evaluate the plan of care and modify as necessary
  5. Adheres to department and company policies, procedures, and methods to increase the department’s contribution to organization effectiveness
  6. Performs related accountabilities/responsibilities as required or directed
  7. Continuously evaluates the client’s status and works with client/caregiver in developing ways to meet their needs.
  8. Coordinates and integrates services being provided to each client by all disciplines.
  9. Initiates and revises client care plan for the caregivers based on client’s needs and supervises the care provided by the caregivers as applicable.
  10. Informs the physician and other healthcare team members of changes in the client’s conditions and needs.
  11. Provides those services requiring skills using the nursing process.
  12. Initiates and maintains a current, individualized plan of treatment based on ongoing assessments in conjunction with physician and client/caregiver
  13. Initiates and performs teaching of the client, family, and other health team members, as appropriate, in order to meet identified needs and achieve goals
  14. Gives medications according to company policy
  15. Informs clients of their rights and responsibilities and respects these rights and responsibilities
  16. Adheres to all company documentation and timeliness requirements
  17. Complies with all company policies and procedures
  18. Participates in in-service programs as required by policy
  19. Participates in team conferences as required
  20. Participates in Quality Assessment/Improvement activities as indicated
  21. Supervises and teaches other nursing personnel as appropriate
  22. Participates in committees as requested
  23. Efficient client and caregiver handling, as demonstrated by key indicators like client satisfaction and turnover rate.

QUALIFICATIONS

  • Graduate of an accredited school of Nursing.
  • Must be licensed as a Registered Nurse or Licensed Practical Nurse by the Illinois State Board of Nurses.
  • Minimum of one (1) year of nursing experience required. Home health, home nursing experience preferred.
  • Current CPR certification
  • Strong computer literacy, including proficiency in Microsoft Office and Google Workspaces.
  • Ability to work in the field, operate a motor vehicle, have a current, valid driver’s license and be insurable
  • Effective oral and written communication and presentation skills. 
  • Excellent interpersonal and coaching skills. 
  • Excellent organizational skills.
  • Positive, professional, business image.
  • Ability to listen to and understand information and ideas presented through spoken words.
