Schedule: Mon-Fri (8a-5p) Exempt* 

The Nurse Navigator is a Registered Nurse who facilitates patient (s) continuity of care with the healthcare team in an efficient and timely manner. The RN acts in an intake model, assesses the patient needs, assist with planning, organizing, implementing and evaluating the patient care needs with the healthcare team. The Nurse Navigator functions autonomously and in collaboration with other members of the healthcare team as a patient advocate, educator and coordinator. The Nurse Navigator possesses strong verbal and written communication skills.

ESSENTIAL FUNCTIONS OF THE ROLE

In an intake model, the Nurse Navigator utilizes the nursing process of assessment, planning, implementation, and evaluation of patient according to the Physician orders and establishes a plan of care. 

Coordinate with physician's office the testing to be performed in order to make a definitive diagnosis. Coordinate scheduling for outpatient testing. Communicate with all members of healthcare team involved in providing patient care. 

Facilitate timely access to quality medical care from diagnosis through survivorship. 

Collaborates with physicians and other health care providers to improve patient care within an interdisciplinary model.

Educate patient and family members on diagnosis, treatment options and available services. 

Provide direction and support to the patient/family and serve as patient advocate.

Maintain accurate and complete nursing documentation, patient information in Patient Navigator database.

Nurse Navigator will be a point of contact for the patient throughout the continuum of care.

Participate in case conference where appropriate to maintain and increase disease specific knowledge base.

Participate in continuous quality improvement projects in order to better serve the patient, family and healthcare system to improve the quality of service provided. 

Utilize leadership skills such as decision making and delegation to ensure safe patient care is provided in professional manner.

Visit with Physician and/or office staff acting as a liaison between patient and office on a regularly basis.

Greets patient and families. Assist by providing facility information, directions and general information (transportation, hotels, etc).

Provide escorts for patients and family throughout the hospital for places that are difficult to locate.

Follow up with patients on 3 month and 1 year interval.

Participate/attend community service events.

Assist with on-boarding new nurse navigator (s) to the program.
