Job Requirements

Required Skills:

Associates Degree in Nursing or Diploma.
One (1) or more years of nursing experience
Licensed to practice as a Registered Nurse (RN) in the State of Texas
BLS (AHA)

Preferred Skills:

Bachelor’s Degree in Nursing preferred
One (1) or more years of experience in an Acute Care setting preferred

Responsibilities: RN (Registered Nurse) Complete daily electronic forms for each assigned patient within a secured Electronic Medical Record (EMR) System. Provide admission, case management, and follow-up skilled nursing visits for home infusion and infusion center patients. Administer ongoing care/infusions for each patient, and provide necessary follow-up as directed by the Physician and Clinical Manager. Confer with physicians and pharmacies to develop the initial treatment plan based on the physician's orders and the initial patient assessment. Provide hands-on care, management, and evaluation of the care plan and teaching of the patient under a physician based on ongoing assessments and as required by policy/regulation. Report patient care, conditions, and progress to the patient's physician, pharmacy, and Clinical Manager continuously. Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems. Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation. Prepare appropriate medical documentation on all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes. Prepare visit/shift reports, update/summarize patient records, and confer with other healthcare disciplines to provide optimal patient care.
