Summary:

Under direction and guidance of the physician and Director of Neighborhood Health Center, the Neighborhood Health Center LPN is assigned to the Hospital's Primary Care Clinic and accepts the responsibility for the delivery of patient care through the process of assessment, planning, implementation and evaluation as appropriate to the ages of the patients served.

The Neighborhood Health Center LPN works in close cooperation with, and under the direction and guidance of, the physicians assigned to the Primary Care Clinic. The employee maintains established departmental policies and procedures, objectives, quality assurance programs, safety, environmental and infection control standards. Maintains confidentiality of all clinical and staffing information. The employee strives to incorporate the philosophy and mission of the hospital and to comply with the Patient Bill of Rights.

Mount St. Mary's Hospital consists of multiple locations and associate may be assigned as needed.

Responsibilities:

EDUCATION
  • Graduate from accredited school of Practical Nursing required
  • LPN Licensure in New York State
  • Basic Life Support Certification (must possess BLS within sixty (60) days of job offer
  • HIV counseling certification preferred

EXPERIENCE
  • Minimum of either: One (1) year of recent pediatric clinic or pediatric physician's office nursing experience and primary care clinic or physician's office experience as determined by the Hospital; or one (1) year hands-on acute Medical/Surgical/Pediatric hospital experience as determined by the hospital.
  • Extensive knowledge of third party resource utilization and referral requirements required
  • Two (2) years adult medicine physician office/clinic experience preferred
