JOB DESCRIPTION:

SUMMARY OF POSITION

Provides nursing care to clients in crisis in the Walk-In Center and Crisis Stabilization Unit.   Provides medication monitoring, adjusts medications within limits prescribed by authorized prescriber.   Provides information, education, and support to individuals in crisis that promote wellness and recovery. 

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Performs nursing assessments on individuals being admitted to the WIC/CSU. 
  • Monitors vitals, performs lab testing, and evaluates medical appropriateness for admission and continued stay. Obtain and enters orders into EMR from authorized prescriber.
  • Administers and gives medications to patients, monitors the dosage, side effects, use and documents patients’ reactions. 
  • Provides medication training to the guest or guest’s family/guardian to administer the guest’s medication, to monitor proper levels of dosage and watch for side effects.
  • Develops and maintains the Treatment Plan of Care for guests. 
  • Oversees the administering of nursing needs of guests.
  • Performs as a liaison role between guest and the authorized prescriber, pharmacist, family, and/or collateral support systems.
  • Coordinates guest- staff assignment of care and provides shift change report. 
  • Observes guests on a day-to-day basis and provides documentation in the EMR.
  • Provides group or individual education as needed.
  • Participates and/or leads staff meetings and training as scheduled or necessary.

KNOWLEDGE, SKILLS & ABILITIES

  • Effectively communicate via written, verbal, in person and virtual methods.
  • Performs basic electronic medical record computer skills.
  • Detail oriented with effective problem solving skills
  • Possesses leadership skills
  • Knowledge base in psychotropic medications.
  • Skill in crisis intervention.
  • Ability to de-escalate patient/clients using evidence based de-escalation training (e.g. Theapeutic Options) to include safety holds as necessary.

QUALIFICATIONS

Education Level              

Minimum of Associates Nursing Degree from an accredited school or a Waiver approved LPN by State of TN. RN Preferred.

Years of Experience       

One year of behavioral health nursing experience preferred

Certification/Licensure 

Licensure required. Valid driver’s license required.

Behavior Health Registered Nurse - PRN

Under the direction of the Clinical Nurse Manager, provides nursing care in accordance with departmental and hospital policies and procedures. Essential Functions: Demonstrates knowledge of hospital policies and procedures for administering and recording medication. Demonstrates knowledge of commonly used meds: action, dose, and side effects. Observe five R’s for administering meds: right patient, right drug, right method, right dose, and right time. Demonstrates proper procedures in all methods of medication administration. Maintains IV access according to hospital policy and procedures. Assesses and documents IV site every eight hours per unit standard. Reports change in patient status immediately. Reports the progress of the patient’s/significant other’s response to teaching plan to treatment team. Implements changes in plan of care. Patient is oriented to unit and vital signs recorded within one hour of admission. Each identified problem is addressed using BIRP format every twelve hours and PRN (if applicable). Flow sheets and I & O sheets are signed with name and title. Medication administration record must include full signature and title of nurse administering med, time given, reason for omission, and site for infection (if applicable). Case Management Registered Nurse - PRN

Jennie Stuart Health is seeking a dedicated Case Management Nurse to join our healthcare team on a PRN basis. This role is integral to our patient care system, coordinating services through a multi-disciplinary process that provides both clinical and psychosocial support throughout the continuum of care. The Case Management Nurse will be based at our Medical Center location, working in a mid-career level position with an hourly compensation ranging from $23.02 to $35.97. As a Case Management Nurse at Jennie Stuart Health, you will be responsible for developing, planning, coordinating, and administering activities related to clinical review, discharge planning, resource utilization, and utilization review. Your primary goal will be to ensure quality, cost-effective care for our patient population, embodying the core values of patient advocacy, holistic assessment, care planning, and empowerment of patients and families. In this role, you will build collaborative relationships with staff, peers, and physicians while communicating with clarity, integrity, and purpose to optimize healthcare outcomes. You will assess the medical necessity of all admissions, validating the need for hospitalization under the correct patient status, and competently evaluate the needs of patients and their families from admission throughout their hospital stay. A critical aspect of this position involves proactive discharge planning beginning on the day of admission, which includes patient/family education and the development of safe, quality-driven post-discharge plans. You will collaborate with the multi-disciplinary team to develop and implement appropriate discharge plans for assigned patients and assist in monitoring and evaluating patient/family progress according to established goals. As our Case Management Nurse, you will serve as an effective liaison between our facility and payers to optimize patient/family reintegration into the community. You will coordinate the integration of various social services into all aspects of patient care and ensure documentation requirements meet facility, state, and federal guidelines. Your documentation should reflect psychosocial and medical background, including high-risk screenings related to case management and proposed interventions/outcomes. This position requires creative problem-solving skills, utilizing innovation and a team approach.
