Job Description: The Palliative Medicine Service Community Based Nurse Practitioner reports to the Palliative Medicine Service Manager and works collaboratively with the Interdisciplinary Palliative Team as well as patient’s larger health care team. Caseload consists of community based palliative patients and their families. The Palliative Nurse Practitioner provides specialty palliative care and management of the patient with serious and/or chronic illness, in collaboration with the supervising physician and through implementation of standardized procedures.
