Job Summary: The RN Program Coordinator leads the planning, development and implementation of program goals designed for system wide improvement in the care of patients. The position is responsible for educating, coordinating and navigating patients through the JPS Health Network.
Essential Job Functions & Accountabilities:
- Assists with the planning, coordination, implementation and education.
- Coordinates and oversees ongoing performance improvement processes related to respective department.
- Collaborates with medical staff to provide individualized patient care.
- Monitors program metrics, identifies benchmarks and establishes opportunities for improvement.
- Responsible for collecting outcome data and evaluating its effectiveness.
- Provides the organization with appropriate strategies for a successful program; participates in interdisciplinary committees; and others as assigned.
- Serves as a clinical expert and resource for the education of peers, staff members, and other health professionals.
- Participate in research studies as needed, adhering to all policies of the Institutional Review Board.
- Develops and provides education to patients and their families about the program, advance care planning and services available at JPS Health Network.
- Services as a liaison between the inpatient/outpatient flow of patients. Coordinate outpatient clinic schedule according to current inpatient census. Assists in scheduling outpatient follow-up appointments for patients as they are discharged from inpatient.
- Develops and recommends protocols and guidelines for respective program and best practices.
- May be required to provide nursing interventions where appropriate: fluid and/or medication administration, EKGs, laboratory testing (i.e. blood draws, urine collection).
- Assists with special projects across the JPS Network as assigned by leadership.
