Job Details

 

Our Telehealth LPNs coordinate the Telehealth program for Catholic Home Care to insure the optimal utilization of the technology to enhance patient care and clinical excellence.  Adhering to Catholic Home Care department policies and procedures and TJC/regulatory agency requirements.  Demonstrates and promotes service excellence at all times.

 

Experience and Skills:

  • Strong clinical skills and 2-3 years recent hospital/home care experience
  • Strong communication and interpersonal skills
  • Computer Skills
  • Acute care telemetry or triage experience is preferred

Licensure:

  • Currently Licensed by New York State as a Licensed Practical Nurse (LPN)


Salary Range

USD $30.00 - USD $34.00 /Hr.



This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.

Describe responsibility: Skills Competency• Demonstrate knowledge and competency according to the LPN role, standards of practice, and licensure. This includes all care delivery and patient management. Qualifications - Graduate from an accredited LPN program - Active NYS LPN licensure (Legally Required) - Current BLS certification (Policy Requirement) - High School or GED (Required) - Graduate of an Accredited Nursing school for Licensed Practical Nurse (Required) - Knowledge of EMR systems/programs (Required) - Microsoft Office Suite (Preferred)
