Details

The Nurse Practitioner performs physical examinations and preventive health measures within prescribed guidelines and instructions of physician.

Records physical findings, and formulates plan and prognosis, based on patient's condition.
Discusses case with physician and other health professionals to prepare comprehensive patient care plan.
Prescribes or recommends drugs or other forms of treatment.
Monitors and maintains progress of patients and reports progress to supervising physician.
Has the ability to work collaboratively with hospice employees and volunteers as part of an IDT

Qualifications

While performing the duties of this position, the employee is regularly required to assist in lifting and transferring patients as required.
Specific vision abilities required by this job include: close vision, distance vision and ability to adjust focus.
Requires completion of accredited Nurse Practitioner program.
State licensure in good standing
Must be willing to drive with reliable transportation, valid driver’s license, and auto insurance

The Pediatric NP Provider Hospice/Palliative Care has two primary responsibilities. He/she will provide NP-appropriate service to patients enrolled in hospice and may also assist in staffing pediatric palliative care consultation services that function as a program of MJHS Hospice and Palliative Care. He/she may work in a community, long-term care environment, or hospital. He/she has specialist skills in symptom management, communication including goal setting discussions, and hospice eligibility review, and he/she may work independently or as part of a treatment team. In all settings, he/she will provide in-person visits or telehealth visits, as appropriate. Care will consistently comport with best practices, including history-taking, appropriate examination, formulation and care planning, treatment recommendation or implementation, communication, and documentation. In the role as hospice NP, he/she will coordinate with the team physician and manager and will be responsible for face-to-face evaluations for hospice eligibility, and the assessment and management of hospice patients in need of exigent medical care. He/she will also contribute to the training of the hospice team, providing support for the interdisciplinary team, as needed. He/she will also help build a teaching curriculum on pediatric aspects of hospice care that will be of use to our hospice workers who are engaged in pediatric care through cross coverage or for when young adolescents are incorporated into adult hospice teams.
