Description: The Registered Nurse (RN) Congestive Heart Failure (CHF) Program Coordinator is responsible for coordinating care for congestive heart failure patients receiving care at JPS. The RN Program Coordinator is responsible for performing assessments, collecting data and collaborating with other members of the health care team to prepare patients for hospital discharge, assist with an effective transition to the ambulatory setting and provides patient, caregiver and community education as requested. The RN Program Coordinator collaborates with other professional disciplines to ensure safe, effective, and efficient patient care delivery and the achievement of established patient outcomes.
Typical Duties:
- Coordinates all components of the JPS Congestive Heart Failure Program.
- Provides patient, caregiver, and community education as required in a variety of settings, inpatient, ambulatory, classroom, etc. May assist in the development of patient, caregiver and staff education materials and course curriculum.
- Schedules post-hospital discharge clinic appointments and CHF education class appointments.
- Participates in regular meetings and process improvement initiatives with providers, clinic leadership, and other team members. Assists with developing and implementing measures that will enhance patient care.
- Delivers nursing care to congestive heart failure patients with an emphasis on education by identifying and addressing knowledge deficits, treatment planning, pain management, and discharge planning utilizing the nursing process.
- Conducts, analyzes, and reports on quality indicators related to the management of the congestive heart failure patients' care at JPS.
- Serves as the primary point of contact for clients and others. Links patients to appropriate care providers and or community resources to increase patient ability to manage their healthcare effectively.
- Serves as patient and caregiver advocate.
- Adheres to established productivity standards
- Collaborates with the health care team, contributing appropriate recommendations, strategies, and interventions in the interdisciplinary team conferences for complex patients that need intensive care coordination.
- Assesses patient’s physical and psychosocial needs and makes referrals as appropriate. Coordinates patient care across the network and appropriate organizations by coordinating services with Social Services, Behavioral Health, Nutrition Services, and any other organization that would be of help to the patient by facilitating all necessary diagnostic processes by assisting with barriers such as lack of insurance, financial issues, need for personal support, language, and any other problems.
- Documents patient care in the electronic medical record. Keeps records/reports as needed for the JPS CHF program, including logs, statistics, etc.
- Interfaces with professional organizations. Serves on committees and task forces when requested. Presents quality statistics as required to appropriate groups.
- Delivers quality nursing care while minimizing the risk of infection, contamination, harm, or injury. Maintains safe and clean working environment ensuring compliance with procedures, rules, and regulations.
- Performs other related duties as assigned.
