Overview

Plans and manages patient care according to each patient's needs. Interviews patients and records their medical history and physical condition. Obtains patient vital signs, including pulse, blood pressure, temperature, and respiration. Provides routine care for patients.

Responsibilities

  • Provide 1:1 care for client always following client’s plan of care while maintaining client's dignity and independence
  • Monitoring clients’ vital statistics and reporting abnormal findings to the case manager and physician assessments
  • Performs medication administration, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
  • Complete routines as established by other care professionals such as Physical Therapist, Occupational Therapist, and Speech Therapist
  • Ensures infection control policies are maintained at all times, reports possible communicable diseases or infections, and ensures the overall implementation and practice of infection control procedures are implemented
  • Accurately documents nursing actions of all care given and communicates with family, case manager, and physicians as well as documents the progress and outcomes for established goals and informs physician, case manager, and families of changes in client's medical condition and needs
  • Orders supplies and medications, as needed, to ensure adequate amounts are on hand at all times
  • Ensures all patient needs are being met in accordance with the instructions of the physician.
  • Monitors incidents involving individuals. Ensures incident reports are completed timely, accurately, and case managers are informed; examines the individual following the incident
  • Ensures all medical equipment is maintained and reports faulty equipment to Durable Medical Equipment for repair
  • Other duties as assigned

Qualifications

  • Completed post-secondary coursework to facilitate licensure as a Licensed Practical Nurse or Licensed Vocational Nurse
  • Knowledge of individual service plans, including understanding of client rights and health needs
  • Proof of current license as a Licensed Practical Nurse or Licensed Vocational Nurse in good standing from the state(s) of practice
  • CPR Certified
  • Excellent interpersonal skills, including customer service, patience, empathy, and compassion
  • In-depth understanding of patient care policies and regulations
  • Familiarity with symptoms and signs of improvement for several medical conditions
  • Moderate amount of travel between facilities may be required

SUMMARY: The LPN is responsible for providing direct nursing care to the residents, and supervising the day-to-day nursing activities performed by nursing assistants. ESSENTIAL DUTIES AND RESPONSIBILITIES: Direct the day-to-day functions of the nursing assistants. Meet with your assigned nursing staff, as well as support personnel, in planning the shift’s services, programs, and activities. Make written & oral reports/recommendations concerning the activities of the shift as required. Admit, transfer, and discharge residents as required. Provide leadership to nursing personnel assigned to your unit/shift. Complete & file required record-keeping forms/charts upon the resident’s admission, transfer, and/or discharge. Receive & transcribe telephone orders from physicians & record them on the Physician’s Order Form. Chart nurse’s notes in an informative & descriptive manner that reflects the care provided to the resident, as well as the resident’s response to the care. Fill out and complete accident/incident reports & submit them to Director as required. Perform routine charting duties as required & in accordance with established charting & documentation policies & procedures. Prepare & administer medications as ordered by the physician. Develop & maintain a good working rapport with inter-departmental personnel, as well as other departments within the facility to ensure that nursing services and activities can be adequately maintained to meet the needs of the residents. Create and maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the unit and shift. Monitor your assigned personnel to ensure that they are following established safety regulations in the use of equipment and supplies. Maintain confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information.
