Track, order completion and follow up. Monitor patient compliance with patient treatment plan and assist patients with referrals to outside resources and care coordination as needed. Serve as the central point of contact for all oncology navigated patients, including interactions with medical, nursing ancillary service providers. Provide education to patients, families, and significant others via phone or written consultation. Demonstrate efficiency with delegated orders and data entry with oncology EMR. Provide patient support and phone triage skills for the oncology population. Follows Chemotherapy dosing based on National Comprehensive Cancer Network (NCCN) or other Chemotherapy guidelines with coordination of complex chemotherapy regimens with infusion and pharmacy staff. Demonstrates effective communication with other members of the healthcare time, promoting effective team functioning. Assist with daily telephone triage for oncology patients.
