NuRoc Health and Rehab is growing, and we’re excited to invite compassionate and skilled professionals to be a part of our dedicated team! We are currently seeking a full-time MDS Coordinator.

About the Role:

As our MDS Coordinator, you will play a key role in assessing resident health, collecting and analyzing data, and developing individualized care plans that align with each resident’s needs and expectations. This position is ideal for an experienced nurse who enjoys a combination of clinical and administrative responsibilities, with a focus on quality care and compliance.

Key Responsibilities:

Conduct comprehensive assessments of residents
Develop and update resident care plans
Monitor patient care through assessments, observation, and medical record reviews
Collaborate with clinical teams to ensure accurate documentation and effective care delivery
Create and manage medical codes for billing and recordkeeping

Qualifications:

Registered Nurse (RN) licensed in the state of Wisconsin
Previous MDS experience in long-term care is preferred
Strong communication, organization, and analytical skills

Skills/Qualities: Must be dedicated to learning and possess adaptable skills - The RAI manual is constantly being updated and revised according to Medicare requirements. Critical thinking skills with the use of expert clinical skills to determine resident condition and level of care needed. Able to determine significant changes in the clinical picture. Minimum Qualifications for the Job: Education: Graduation from an accredited school of nursing, with Bachelor of Science in Nursing preferred.
