Job Summary and Responsibilities

 

Glendale Memorial Hospital is seeking a skilled Inpatient Rehab Facility (IRF) Coordinator to optimize our patient admissions and education. This critical role involves conducting thorough assessments for Acute Rehab Unit (ARU) placement, providing essential guidance to healthcare professionals on medical necessity documentation and functional improvement metrics, and ensuring all regulatory reports are accurate and submitted promptly.

 

  • Collaboration, Dignity, Excellence, Justice, and Stewardship. Demonstrates a positive attitude, takes ownership, and fosters collaborative communication.
  • Conducts pre-admission evaluations to determine patient appropriateness for the Acute Rehab Unit (ARU), assessing medical necessity, participation willingness/ability, and discharge plans. Recognizes and addresses patient/family cultural and special needs, seeking guidance when necessary.
  • Facilitates admissions of appropriate patients to the ARU from GMHHC acute. Talks with Patient Flow Coordinator to see if there are patients for Rehab.
  • Facilitates and reviews documentation for accurate Functional Improvement Measures (FIM) scoring, providing feedback to care providers and escalating non-compliance issues.
  • Serves as a FIM expert, educating all clinical disciplines on patient assessment, documentation to ensure valid FIM scores, and understanding its critical role in IRF PAI and reimbursement.

 

This position is represented by CNA  and is covered by the terms and conditions of the applicable collective bargaining agreement.

Job Requirements

Required

  • Current and Active CA, RN License required.
  • One year experience with Functional Improvement Measure (FIM) assessment preferred.
  • American Heart Association Basic Life Support Certification (AHA BLS). 
  • LA City Fire Card (obtain within 90 days of hire) required.
