Job Summary and Responsibilities

As a Navigator RN, you will provide expert nursing guidance and support to patients as they navigate complex healthcare journeys.

Every day you will assess patient needs, educate on treatment options, coordinate care across multiple providers, and facilitate access to resources, acting as a central point of contact.

To be successful, you will demonstrate strong clinical knowledge, exceptional communication and problem-solving skills, and a compassionate, patient-centered approach to ensure seamless and informed care.

Essential Functions:

Coordinates care for all assigned patients throughout the continuum of care from diagnosis to release from care. Develops and shares assessments, goals, nursing interventions and patient response; remains a support system throughout the patient’s course of treatment.
Provides education, psychosocial and logistical support to improve patient preparedness for treatment and to streamline care path transitions
Coordinates tests and other procedures so all are timely and with as few interruptions to the patient’s life as possible
Attends and participates in hospital patient rounds with physicians and/or multi-disciplinary teams, depending on the assigned department.
Consults with physicians/providers who need assistance with the care of their patients related to continuity of care, coordination of care and education, and resources services available.
Refers patients to appropriate resources/professionals, and coordinates the appropriate resources and consultative services to provide seamless continuity of care and appropriate follow-up plan of care.

Compensation Range: $37.50/hr-$56.00/hr Schedule: 7pm-7:30am Roles and Responsibilities: Provides and coordinates care by assessing physical and behavioral health needs of client, develops and implements nursing care plans, maintains medical records and educates clients and their families about various physical and behavioral health conditions while ensuring client safety at all times. Assesses client’s status on an ongoing basis and pertinent information gathered is documented. Ensures nursing treatment plans are specific to assessed needs and initial treatment plan is completed upon admission. Takes physician’s orders (verbal or via telephone) accurately and documents appropriately; transcribes and reviews physician’s orders. Ensures physician’s orders are legible if taken verbally and transcribed appropriately per policy and procedure. Accurately administers medications per ordered note and documents accurate count of controlled substance. Adjusts medications under provider’s supervision as needed; monitors lab results to validate medication compliance. Notifies physician of significant changes in status such as difficulties with detox, medical emergency or change in medical status regarding response to medications, etc. Supervises self-administration of medications using safety initiative of using two means of client identification: documents and reports effects of scheduled and PRN medications. Maintains accountability of administration, recording, control and security of medications, including narcotics. Completes admission and discharge processes efficiently and accurately as directed. Confers regularly with treatment team (e.g., nurses, therapists, program managers, physician) regarding client care. Attends and participates in the unit activities such as therapy groups, family meetings as appropriate, staffing, unit meetings, etc. Provides education/explanation to clients, families, and co-workers regarding medication usage/maintenance, common side effects, client's rights, as well as other topics necessary for optimal client care. Monitors client acuity, census, staffing needs, and makes arrangements for coverage as needed, following established guidelines. Assures thorough, accurate documentation is completed for all clients on each shift and client assignment sheets are completed daily and tasks are carried out as assigned. Gives thorough report to oncoming staff, supervisors, and physician. Provides coverage in emergency services as needed. Ensures the development, implementation, and evaluation of policies, programs, and services consistent with the facility’s strategic goals and objectives. Roles and Responsibilities: ESSENTIAL FUNCTIONS: Assists DON in the management of the daily operations of nursing services, developing, maintaining and implementing compliant nursing services, policies and procedures. Interprets operational indicators to detect census and acuity changes and increases or decreases in volume that could impact staffing levels, revenues or expenses. Acts to correct undesirable trends. Allocates nursing resources based on measurement of client acuity/care needed.
