MDS Coordinator Job Summary:
We are seeking a compassionate and detail-oriented MDS Coordinator to join our interdisciplinary team at the Westchester Center for Rehabilitation & Nursing, a leading provider of rehabilitative therapy and long-term care in Mount Vernon, New York. As an MDS Coordinator, you will play a critical role in managing the minimum data set (MDS) assessment process, ensuring accurate and timely completion of resident assessments, and facilitating communication among healthcare team members.
MDS Coordinator Responsibilities:
Coordinate the completion of MDS assessments for all residents, ensuring compliance with federal and state regulations
Review and analyze MDS data to identify trends, issues, and areas for improvement
Collaborate with healthcare team members to develop and implement individualized care plans
Facilitate communication among caregiving staff, family members, and healthcare providers to ensure continuity of care
Maintain accurate and up-to-date records, including resident assessments, care plans, and progress notes
Participate in quality improvement initiatives to enhance resident care and outcomes
Provide education and support to staff on MDS assessment and care planning processes
MDS Coordinator Requirements:
Current RN license in the State of New York (RN preferred).
Minimum of 2 years of experience in a skilled nursing facility, with at least 1 year of MDS coordination experience preferred.
Strong knowledge of CMS regulations, RAI process, and PDPM.
Excellent organizational, communication, and problem-solving skills.
Ability to work collaboratively in a team environment.
Proficiency in electronic medical record systems (experience with SigmaCare/MatrixCare a plus).
