This job provides individualized, direct nursing care of assigned patients under the direction of a registered nurse, mid-level provider or physician. Assists in data collection and implementation of the delivery of health care services, and assists other members of the healthcare team in providing the highest quality personalized patient care. Maintains a positive work relationship with the health care team, and exhibits the skills to provide care and service with individuals of all assigned age groups.

Education

Required - Graduate of an accredited school of practical nursing.

Work Experience

Required - 2 years of LPN experience

OR

1 year of LPN experience with 1 year supervisory experience.

Preferred - Experience in ambulatory care.

 

Certifications

Required - Current licensed practical nursing (LPN) license in state of practice.

Basic Life Support (BLS) from the American Heart Association.

Knowledge Skills and Abilities (KSAs)

  • Proficiency in using computers, software, and web-based applications.
  • Effective verbal and written communication skills and ability to present information clearly and professionally to varying levels of individuals throughout the patient care process.
