Be the Best Clinical Support Specialist You Can Be 

If you meet these qualifications, we would love to meet you: 

  • LVN/LPN with current licensure to practice nursing in the practicing state.
  • Possess and maintain CPR certification.
  • Previous experience in home care setting preferred.
  • Must be a licensed driver with an automobile that is insured and in accordance with state and/or organization requirements and is in good working order.
  • Prior use of Electronic Medical Records (EMR) preferred.

Responsibilities:  

  • Daily completion of the following tasks: daily management of KPIs, field staff support, verbal orders, workflow, escalation/complaint calls, updating clinical information, and field visits as required.
  • Stand up / Stand down & Case Conference required reporting to ensure quality outcomes and exceptional patient experience.
  • Reporting and management of patient episodes that are at risk for LUPA status – including recognition of risk, understanding missed visit concerns, coordinating with clinicians/patients/schedulers as needed, etc.
  • Reporting and management of case management model with focus on paraprofessional utilization – including correct visit types selected, continuity of clinical team, capacity management, etc.
  • Maintain clinician productivity within organization standard parameters (defined in KPIs) – including tenured productivity to target, recent hire productivity, etc.
  • Monitor workflows to ensure SLA are met for sync time, face-to-face encounters, etc

Administer all treatments as ordered by physicians. Notify physician of changes in status of resident that might require a treatment change. Make rounds with physician as needed. Transcribe all physician orders. Telephone or fax physician as needed for medication or treatment changes. Follow-up on these faxes or calls. Order medications from pharmacy as needed. Provide accurate, daily charting as necessary on pertinent charts. Be aware of the legal and ethical implications of your duty. Be courteous to residents and residents’ families and respond in an appropriate professional manner as the situation requires. Follow-up on all lab work. Know when a situation cannot be handled and be willing to ask for help and know who to ask. Abide by the policies of the facility and ascertain that employees under his/her direction do the same. Supervise and document serving of prescribed diets and fluid intakes and report persistent unresolved problems to the physician and/or Director of Nurses. Preparation and 72-hour follow-up of Accident and Incident Reports. Ensure that drugs covered by the Controlled Substances Act are verified by inventory and count each shift and any discrepancy immediately reported to the Administrator and/or Director of Nurses. Detect and correct situations that might injure a resident or employee. Ensure continuing promotion of the resident’s physical and emotional health by assisting with his/her medical care.
