The RN MDS Coordinator is responsible for accurately completing assessments and care plans that comply with federal and state regulations, identifying resident needs, and communicating these to families. They develop care plans, oversee restorative nursing aides, attend care plan meetings, and manage data entry and troubleshooting. The role requires competence in general nursing skills for adult and geriatric patients, ensuring resident safety and comfort, and may include acting as a staff nurse or participating in weekend coverage. Additional duties are assigned by leadership to support resident care and facility operations.

Requirements:
36 hours per week - First Shift
Monday - Friday
8:00 AM - 4:30 PM
On - Call Rotation
Current RN license in the State of Indiana
CPR Certification
Knowledge of the MDS and total Care Planning process is required.
Must have good clinical, assessment, and data entry skills.
