Your Role:
As a Hospice RN, you’ll be a vital part of our interdisciplinary team, providing skilled nursing care, emotional support, and guidance to patients and their loved ones. Your responsibilities will include:

Conducting patient assessments and developing individualized care plans
Administering medications and managing symptoms
Collaborating with physicians, social workers, and chaplains
Educating families on care techniques and what to expect
Advocating for patient comfort and dignity at every stage

What You Bring:

A valid RN license in Pennsylvania
CPR certification
A heart for hospice care – previous experience is helpful, but not required
Strong communication skills and a gentle, empathetic approach
Reliable transportation for home visits

Responsibilities Performs initial nursing assessment on new admissions. Completes , coordinates, and revises the total plan of care and maintains continuity of patient care by collaborating with appropriate staff. Regularly communicates patient progress to the clinical manager, physician, and care team. Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies. Records pain/symptom management changes/outcomes as appropriate. Educates families on patient condition, needs and care. On-call requirements Each day our 95-bed ED treats 300 to 400 patients with chronic and acute issues, ruling out life threatening conditions and intervening when necessary to stop the progression of acute processes. The team sees everything from sore throats, ear infections, lacerations and broken bones to more serious conditions like stroke, heart attack, cardiac arrest, and traumatic injuries from motor vehicle and farming accidents, gunshot wounds and stabbings.
